Phoenix NP currently only operates in the state of Arizona and licensed as a specialist for Women's Health.

Patients must be female and live in the state of Arizona to receive services.

Our services are HSA/FSA eligible. We are a private pay clinic and do not accept medical insurance

Your Total Costs:

Phoenix NP prescribes only to the respective manufacturer's direct pharmacy to get you the lowest price for the medications, without the need of pre-authorizations or manufacturer coupons. The direct pharmacy will contact you for payment (you pay them) and ship the medications directly to your home!

*Per manufacturer, pricing on the Zepbound 7.5mg - 15mg remain discounted to $449 as long as refills are within 45 days of prior delivery.

Weight loss results will vary for each person.

All plans are automatically renewed and billed as stated on the plan, and can be cancelled at any time. There are no long term commitments required. We strive to always have fair, honest, and transparent billing practices.

 

Refund Policy:

Cancellation Before Initial Consultation: If a patient chooses to cancel their subscription before the Initial Consultation, a non-refundable fee of $20 will be retained by Phoenix NP due to administrative and payment processing costs.

Cancellation After Initial Consultation: If a patient cancels after completing the Initial Consultation visit and the prescription has already been sent to the pharmacy, the entirety of the Weight Loss  subscription plan and the "New Patient Setup & Initial Consultation" charge will be retained.

Unable to accept as a patient: If during the Initial Consultation, the provider determines that you are not a candidate to be prescribed the medications or that they are not able to provide services to you due to a medical condition, then the entire first payment will be refunded.

Recurring subscription renewal cancellation: It is the responsibility of the patient to be aware of when their next subscription renewal will be charged, based on the timeframe listed on the plan. If the patient desires to cancel the subscription before the automatic payment is charged, it is their responsibility to cancel through the website or by notifying Phoenix NP through text, message, phone call, or during an appointment. If the patient notifies Phoenix NP within 48 hours after their payment method is charged AND the prescription renewal has not been submitted yet, then a full refund of that recent renewal charge will given. 

Credit for Plan Change: If a patient decides not to continue with the plan but wishes to switch to another product or service offered by Phoenix NP, if available, the most recent charged amount will be applied as credit toward the selected product or service.
